Registration Form
Thank you for your interest in the NDC Index.
Only one registration is accepted per company.
This registration form should be filled out by the survey coordinator, who will be the primary point of contact for completing the NDC Index survey form.
The submission deadline for the NDC Index is every March 1st, but registration is ongoing. To participate in the current year of the NDC Index,
you must register by February 1.
All registrations received after February 1 will be included in the following year's index.
After filling out this form, you should expect your company's unique URL link in your inbox within a week. Through this link you can access your survey form.
Note that links are only sent between October 1st and February 1st.
If you did not receive an invitation e-mail after this time period and it is not in your spam folder, please contact us.
Note: At this time we are not accepting registrations from higher-education institutions.
Message for 2023 NDC Index Participants:
Companies who participated in the 2023 NDC Index do not have to re-register. When the survey opens on October 1st, your survey coordinator will automatically receive your unique survey link. Only re-register if the your company's survey coordinator has changed. Remember that the survey coordinator will receive your organization's survey link.
